3. No Emergency Care: Telehealth services through Prescribey are not intended for emergency situations. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
4. Limitations: While telehealth provides access to care, it may not always replace a traditional in-person evaluation, and some conditions may require follow-up in a clinical setting.

Acknowledgements

By using our services, you confirm the following:

• You are at least 18 years old and are requesting care on your own behalf.
• You have reviewed and understand the risks and limitations of receiving healthcare through telehealth.
• You will provide complete and accurate health information to assist in the evaluation and treatment process.
• You understand that your healthcare provider will determine whether a prescription is appropriate based on your health profile and consultation.

Prescribey LLC complies with HIPAA and applicable privacy laws. Your health data is protected and used only for purposes of treatment, care coordination, and service administration.

Consent to Treat

By signing electronically or proceeding with our service, you voluntarily consent to receive care via telehealth from providers working with or through Prescribey LLC.
